What Insurance Typically Covers
Most major insurance plans — including Medicare — cover interventional pain management procedures when medically necessary. Covered services typically include:
- ✓Epidural steroid injections — for herniated discs, sciatica, spinal stenosis
- ✓Radiofrequency ablation (RFA) — for facet joint arthritis, chronic back & neck pain
- ✓Nerve blocks — diagnostic and therapeutic nerve blocks
- ✓Joint injections — knee, hip, shoulder, sacroiliac joint
- ✓Spinal cord stimulation — trial and permanent implant (prior auth required)
- ✓Office consultations — initial evaluation and follow-up visits
- ✓Diagnostic imaging — X-ray and MRI interpretation
PRP (platelet-rich plasma) therapy is typically not covered by insurance as it is considered investigational by most carriers. Ask our team about self-pay pricing.
How We Handle Your Insurance
We Verify Your Benefits
Before your first appointment, our team contacts your insurance carrier directly to confirm coverage, deductible, copay, and any referral requirements.
We Obtain Prior Authorization
For procedures requiring prior auth — epidural injections, RFA, spinal cord stimulation — we manage the entire approval process. You won’t need to handle this yourself.
We Submit Claims on Your Behalf
After each visit or procedure, we submit claims directly to your insurance company and follow up on denials and appeals as needed.
You Receive a Clear Statement
Once insurance processes the claim, you receive a simple statement showing any remaining patient responsibility — typically just your copay or coinsurance amount.
Medicare & Medicare Advantage
Dr. Qureshi is a participating Medicare provider. Traditional Medicare Part B covers most pain management procedures — including epidural steroid injections, radiofrequency ablation, nerve blocks, and joint injections — when medically documented and necessary.
Important: Medicare requires documentation of prior conservative treatment (physical therapy, medications) and supporting imaging (MRI or X-ray) before authorizing interventional procedures. Dr. Qureshi will review your history and ensure everything is properly documented to support your claim.
Workers’ Compensation
If you were injured on the job and are receiving workers’ compensation benefits in Texas, our office treats work-related musculoskeletal injuries, spine injuries, and chronic pain conditions. We work directly with your claims adjuster and case manager.
Self-Pay & Uninsured Patients
We offer competitive self-pay rates for consultations and procedures. Our team will provide a clear cost estimate before any visit or procedure. We want cost to never be a barrier to quality pain care.
